We are currently in network with Aetna, Cigna and United Healthcare (UHC) insurance. 

Nutrition counseling is covered under the affordable care act as preventive medicine so many insurance companies will cover nutrition visits in full as preventive, however, each insurance plan has its own nuances in terms of coverage.  This is why it is required to contact your insurance company prior to your first visit to verify your coverage.

How to Verify Your Insurance Coverage

If  you would like to verify your benefit coverage for “Nutritional Counseling” and “Medical Nutritional Counseling” with your insurance, here are details on what to ask.

*If you are calling about an insurance that is out of network, please call your insurance and inform them you will be seeing an out of network provider and then proceed to questions 3-7.

  1. Call the patient line or customer service number listed on the back of your insurance card
  2. Verify that we are in network for your plan by providing your Nutritionists NPI (1700650587).
  3. Ask if the following CPT codes are covered 97802 (initial appointment) & 97803 (follow-up appointments). If they say you do not have coverage using those codes, THEN ask them to check your coverage for the following CPT codes: 99401, 99402, 99403, 99404.
  4. Ask if the ICD10 diagnostic code Z71.3 is covered without any other diagnosis or if a secondary medical diagnosis is also required
    1. If another diagnosis is required ask what diagnoses or diagnostic codes are covered under your plan (cardiovascular, diabetes, obesity, etc).
    2. If you have one of the diagnoses, we will need a direct referral from your doctor with the ICD10 code for that diagnosis.  Ask if there is a specific diagnosis or BMI needed on your primary care physician referral. We will need this faxed to us before your appointment (fax #: (475) 271-4403)
  5. Ask if there is a limit to the sessions covered; what is your deductible and what is your specialist co-pay?
  6. Ask if prior approval is required for additional sessions if medically necessary?
  7. Get a reference code for your conversation with the representative you speak with at your insurance company.
